Always Care For You (AC4U) provides services that allow a person with special needs to stay in their home. The care can be for people who are getting older, are chronically ill, recovering from surgery, disabled, or any person that needs in-home care.

Home Care Service Includes

Community Care Services Program

Overview

The Community Care Services Program (CCSP) is a Medicaid home and community based waiver services program that provides community-based social, health and support services to eligible consumers as an alternative to placement in a nursing home. The Georgia Department of Community Health’s (DCH) Division of Medicaid contracts with Georgia’s 12 Area Agencies on Aging (AAAs) to administer the program statewide.

*What is a Medicaid Waiver?

For persons with limited financial resources, Medicaid pays for nursing home care. For those who wish to live at home or in assisted living, sometimes Medicaid will pay for care in those locations if it can be obtained at a lower cost than in a nursing home. It does this through “Medicaid Waivers.”

SOURCE is an acronym for Service Options Using Resources in a Community Environment. The program is designed for frail, elderly, and disabled Georgians who require the level of care typically provided in a nursing home. However, this program allows eligible individuals to receive care in their homes or communities (such as in assisted living facilities, also referred to as personal care homes) and avoid having to use long-term nursing home care. For some very low-income participants, this program provides both medical care and non-medical personal care services.

We assist any individual, families, disabled persons, veterans, and senior citizens with everyday essentials from housing to shoes on their feet. Whatever we can assist in is our focus. With your donation proceeds and company funds, Rhema is able to provide food, blankets, sleeping bags, water, clothes, & wireless devices. Housing and Adult Day Centers are our future goals.

Our organization is dedicated to providing support and resources to disabled individuals, seniors, veterans, young men, and women facing challenging circumstances. We offer programs designed to meet the unique needs of each of these groups.
